Bates, Kentucky Vol. Infantry, 1863 (plus one servant; Capt. Geo. A.J. Averlin, 1864 (plus one servant); Capt. Samuel Mayall, posted at Bowling Green, KY, 1862-63 (plus one servant); 1st Sgt.
