Published by the Merchants Publishing Co., Denver, Co. for W. D. Armstrong, Stationer, Cripple Creek, Colo. Small pamphlet glued onto inside back cover: "History of Midland terminal and the Early Days of Cripple Creek. A Story More Interesting than Postcards or Folders Can Describe".
